Tangelo partners with Medicaid managed care plans and Social Care Networks — including EngageWell IPA and Public Health Solutions in New York — and with California health plans including Blue Shield of California, Contra Costa Health Plan, and IEHP, to deliver eligible health-related social needs (eHRSN) nutrition services. Eligibility is determined by your health plan. In California, enrollment requires a referral from your doctor (PCP) or Enhanced Care Management (ECM) team.
